This year all adult members and guests will have lanyards for security purposes. 

• If anyone in your household will be attending ANY High Holiday service in person please indicate the number of people who will be there. In the Notes space, please list those that will be attending for their lanyards.
• If you do not plan on attending any services in person, you do NOT need to complete this form.
